Help Information For Filling Out The ATC Forms


Following is information regarding filling out the name and address fields on the forms.

ATC Number:   Enter the number that has been assigned to you by the ATC Office.

Last Name:   Enter your last name.   When entering your spouses name and the last name is the same, leave the last name on the second line blank.

First Name:   Enter your first name.   A first name must be entered in the first box.

Names:   Enter the First Name of one member, an &, and the First Name of the second member followed by the last name.   If the last names are different, enter the First and Last Name of one member, an &, and the First and Last Name of the second member.

1st Line of Address:   Enter the first line of the address.

2nd Line of Address:   Enter the second line of the address if there is one.

City:   Enter the city.

State:   Enter the first letter of the State or Province.   If the correct name is not shown, repeat the first letter until the correct name is shown.

Zip Code:   Enter the Zip or Postal Code for your mailing.   If the 9 digit Zip Code is entered, enter a dash after the fifth number.

Phone Number:   Enter your phone number.   You do not need to enter the dashes, they will be inserted.

Cell Phone:   Enter your cell phone number.   You do not need to enter the dashes, they will be inserted.

1st E-Mail Address:   Enter your e-mail address.

2nd E-Mail Address:   Enter a second e-mail address is you have one.

Chapter Numbers:   Enter the numbers of the ATC Chapters that you belong to.   If you belong to more than one chapter, separate the numbers with a comma.
